CHILDREN’S MIRACLE NETWORK DONATION FORM
 

For a donation of $100 to the Children’s Miracle Network, we are giving you the opportunity to advertise your business at the North Miami Beach Costco location.  100% of the donations go to the Miami Children’s Hospital. Please stop by to remit payment in person, or fax this form to the fax number below.  BOTH SIDES of the poster  space is 16” x 20” and must be printed at the North Miami Beach Costco Photo Lab for $5.99 each.  Your company information will be on display above all of our cash registers until May 31st!  Posters will are available and need to be completed by May 10th, limited quantities available.  Once payment is received please stop by the warehouse and bring your logo.  Deadline is May 10th, 2013.
